Adult Patient Forms

Complete patient intake package for adult patients (18+ years). Please download, print, and complete all forms before your appointment to expedite your visit.

Our adult intake forms will be available online soon. In the meantime, forms can be completed in-office or emailed to you upon request. Please call our office if you’d like to receive them before your appointment.

Child Patient Forms

Specialized patient intake package for pediatric patients (under 18 years). Parent or guardian must complete all forms prior to the child's appointment.

Online forms for pediatric patients will be available soon. We’re happy to provide them by email prior to your child’s appointment, or you may complete them in-office. Give us a call and we’ll take care of the rest.

Insurance & Payment Information

We accept most major insurance plans and are committed to helping you understand your benefits and coverage options. Our billing team will work with you to maximize your insurance benefits and provide clear information about any out-of-pocket costs.

Please bring your insurance cards to every appointment and inform us of any changes to your coverage. We recommend contacting your insurance provider to verify your eye care benefits before your visit.

Payment Options: We accept cash, check, and all major credit cards. Financing options may be available for certain procedures.

Refractions

A refraction is the process of determining your best possible vision and prescription for glasses or contact lenses. This service is an important part of a comprehensive eye examination, helping us evaluate your overall visual acuity and eye health. Please be advised that:

The refraction service is not typically covered by most medical insurance plans, including Medicare.
Insurance plans often consider refraction a vision-related service, separate from the medical portion of an eye exam, and therefore it is billed separately.
The fee for refraction is $45 and it is due at the time of service.
